  IMMUNIZATIONS

Waukesha County Public Health Division
New Student Immunization Requirements are in effect for student in grades K-1, 6-7, 9-10, and 12.  Tetanus/Pertussis and/or Varicella (chickenpox) vaccines are required, plus Meningitis and HPV are recommended for adolescents.

Medication At School
Any medications that will be administered at school must be accompanied by a pdfdistrict prescription medication form signed by the prescribing physician and parent if it is a prescription medication OR a pdfdistrict non-prescription medication form signed by parent if it’s a non-prescription medication.
 
If there is a dose change during the school year on any prescription medication, a new medication form must be signed by the prescribing physician .
All medication sent to school must be in the original container or prescription bottle. No medication will be accepted that arrives in baggies or envelopes, etc. Any questions or concerns, contact Mrs. Worth at (262) 679-2300 ext. 4516. Fax# (262) 679-5222.
 
Severe Allergies  ( Bee sting/insect) or Food
 
If your child has a known serious or life threatening allergy where the use of a epi-pen is to be administered, you will need to obtain a prescription  medication form. You may do this by picking one up from the health office or printing one out from this page. Turn in the completed form with the medication at registration time. Having the medication here the first day of school will assure you and your child that if it is needed, it can be given.
If a severe allergy developes during the school year, please notify the health room to make us aware of any special  medical requirements that your child may need.
 
Physical Education Medical Excuse
 
If your child needs to be excused from gym class for a medical reason, a written doctor’s excuse must be obtained and given to the health office. If the medical is open-ended, (example: “Until further notice”) a written release is needed from his Dr. to place him back in gym class. Any medical may be shorten or extended by a written statement from the attending physician
